Who we serve

Five facility types, across L.A. County.

We work with facilities of every shape and scale — small group homes, mid-size assisted living, and larger skilled nursing operations. We adapt to your census, your med-pass workflow, and your resident population.

01

Assisted Living (RCFE)

Multi-dose packaging, monthly cycle fills, on-call pharmacist consultation.

02

Skilled Nursing (SNF)

Acute-care med passes, IV therapy, consultant pharmacist services (CMS-compliant).

03

Adult Group Homes

ARF, ARTF, and developmental disability homes. Custom packaging for cognitive support.

04

Memory Care

Sealed dose packaging, anti-diversion protocols, behavioral medication tracking.

05

Hospice & Palliative

Comfort kits, compounded liquid morphine, expedited dispensing, hospice-specific billing.

What's included

Six services, one contract.

Multi-dose packaging

Each medication is pre-sorted into time-of-day blister cards — color-coded, labeled with patient photo, day, and dose. Designed to reduce administration errors and free nursing time at the med pass.

  • 30-day and 7-day cycle options
  • Photo identification on every card
  • Color-coded by med pass time
  • FDA-compliant tamper-evident sealing

Daily delivery

Routine deliveries six days a week. Same-day emergency delivery when staffing and routes allow — call for current STAT availability and after-hours coverage.

  • Insulated transport for refrigerated meds
  • Signature-required chain of custody
  • STAT orders prioritized when possible — confirm timing by phone
  • After-hours pharmacist on call for urgent clinical questions — call to confirm coverage

e-MAR integration

Coordination with major electronic medication administration record systems via NCPDP and HL7 standards. No double-entry, no faxed orders, no transcription errors.

  • ECP · integration confirmed
  • PointClickCare, MatrixCare, QuickMAR, Yardi, ALIS — on request
  • Real-time refill status updates
  • Automated cycle-fill scheduling

Consultant pharmacist

Quarterly drug regimen reviews per CMS guidelines (F-Tag 757). Each resident's chart is reviewed for therapeutic duplication, dose appropriateness, interactions, and unnecessary medications.

  • CMS-compliant DRR documentation
  • Annual P&T committee participation
  • In-service training for nursing staff
  • State survey preparation support

Direct billing

We bill Medicare Part D, Medi-Cal, hospice insurers, and private payers directly. Your facility doesn't handle paperwork. Residents pay only their copay.

  • Medicare Part D (all plans)
  • Medi-Cal & Medi-Cal Managed Care
  • Hospice/palliative special billing
  • Monthly facility-level reporting

Specialty & compounded

Because we are a full compounding pharmacy in-house, your facility has access to bespoke formulations — flavored suspensions for dysphagia patients, topical compounds, hospice comfort kits.

  • Suspensions for crushing-restricted residents
  • Topical pain compounds
  • Hospice comfort kits
  • Off-market & discontinued meds
Onboarding

From inquiry to first delivery, in 10 days.

We do the heavy lifting of the transition — chart reviews, insurance verification, e-MAR integration. Your residents experience no interruption in care.

Inquiry call

30-minute call to understand your facility, current pharmacy, and resident census.

Facility walk-through

We visit, meet your nursing leadership, review your med-pass workflow.

Chart & insurance review

Our team digitizes every resident's profile and verifies pharmacy coverage.

e-MAR set-up

We integrate with your existing MAR system and run a one-week parallel test.

First cycle fill

Multi-dose packs delivered, in-service training completed, on-call pharmacist assigned.

e-MAR integration

Your med-pass, already in the system.

No faxed order sheets. No transcribed dosing. New orders, refills, and discontinuations flow into your eMAR via standard NCPDP and HL7 messaging — and our system sees them at the same time. Your nurses spend less time on paperwork; your residents get more attention.

Currently on ECP? We're integrated. PointClickCare, MatrixCare, QuickMAR, Yardi, ALIS, or another platform? Call us — in most cases we can confirm the integration path within a few business days.

Why facilities choose TFC

An independent, family-owned, dual-accredited pharmacy serving long-term-care facilities across South LA and the South Bay. The same team that runs the LTC operation also runs an in-house USP <800> hazardous-drug clean-room and a PCAB-accredited specialty compounding lab — so when a resident needs a non-standard formulation or a hospice comfort kit, it's prepared in-house, not outsourced.

Multilingual coordination — English, Spanish, Armenian, Russian — for facilities serving residents and families across LA's diverse populations. Direct billing to Medicare Part D, Medi-Cal, and private payers.
